JOB SUMMARY

The Clinical Assessment and Informatics Lead is responsible and accountable for the oversight of clinical assessment during the admission process and completion of RAI MDS. This involves the direction, planning, organization and coordination of resident assessments to ensure quality resident care, tracking of significant changes, care plan development and RAI coding to ensure the highest quality of care and accuracy to reflect our funding model.

KEY RESPONSIBILITIES :

Administrative Management

· Directs and coordinates all electronic coding and care planning for resident care including the provision of primary health, advanced nursing care and wellness services to meet the Residents' nursing and personal care needs, consistent with the professional Standards of Practice of the College of Nurses of Ontario

· Participates in the clinical assessment of residents upon admission and when significant changes occur

· Works closely with clinical staff to ensure comprehensive care plans are developed using a resident centered care philosophy

· Participates in the nursing services preparations throughout the year for Compliance Review, Case Mix Index (CMI) Review and Accreditation Review

· Works in collaboration with the Clinical Quality Educator to ensure the development of education programs based on resident care needs and staff development

· On learning of any issues, concerns, complaints, initiates immediate action as required. Informs the Administrator, CEO, SLT and Ministry of Long-Term Care, Public Health and any other regulator or office as suitable and necessary

· Participates in clinical committees and committees of management including Professional Advisory Committee

Staff Management

· Models leadership through education and engagement on the floor with staff, assisting and participating in daily duties from time to time

· Immediately informs the Director of Care of any situations or problems that arise that may put UHS or the residents at risk, or that stand in the way of completing responsibilities within quality requirements

· Promotes effective communication, liaison, co-operation and co-ordination of quality management services and programs with community agencies and other health care providers

Health and Safety:

· Carries out Health and Safety practices in accordance with UHS Health and Safety Policies, Emergency protocols and the Occupational Health and Safety Act of Ontario

· Participates in monthly fire drills and Emergency Preparedness drills

Continuing Education

· Responsible to attend all annual mandatory in-services as required by the Ministry of Long-Term Care, and the Occupational Health and Safety Act of Ontario

· Keeps abreast of current therapies and techniques, takes courses where applicable and approved by Administration in order to teach and adapt techniques to meet the needs of the residents or staff

· Maintains current knowledge with current best practices, legislation and regulations of the College of Nurses

KNOWLEDGE, SKILLS and ABILITIES

  • Current registration and in good standing with the College of Nurses
  • Bachelor of Science in nursing degree preferred
  • Current Certificate of Competence, Registered Nurses Ontario
  • A minimum of 3 years' experience in the management of RAI MDS
  • Proven record in clinical care management
  • Current knowledge of the FLTCA, geriatric/long term care best practice and standards and asset
  • Proficiency in computers - Microsoft Office, Work, Excel
  • Demonstrated leadership and management skills
  • Outstanding organizational skills to manage many competing timetables and responsibilities
  • Ability to exercise good judgment, tact and diplomacy
  • Empathetic attitude and desire to contribute towards the wellbeing of the residents
  • Excellent written and verbal communication skills
  • Excellent interpersonal skills

A valid Vulnerable-Sector Police Record Check and a valid 2-step Mantoux (TB) Skin Test are all required as part of the recruitment process.
