Salary Range: $44,000 – $49,300

Summary Description:

This is a blended Care Management position, which includes responsibilities under both the traditional Health Homes Serving Children (HHSC) model and High Fidelity Wraparound (HFW), an evidenced based model of care coordination. In addition to standard care management duties, the HFW Care Manager will provide enhanced, family-driven, youth-guided services to high-risk youth through HFW, supporting families in developing individualized, strengths-based action plans and coordinating intensive wraparound services.

Our Health Home Serving Children Program provides community-based care management services to children from birth to 21 years of age. The Care Manager is responsible for coordinating the development and implementation of an individualized, community-based plan of social, emotional, and medical services for children and adolescents who have special needs. The work focuses on preventing out of home placements, emergency room visits, and inpatient hospitalizations. The Care Manager provides direct client care services in the community and/or video/phone. The Care Manager will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making and collaboration. When possible, the Care Manager will utilize evidence-based practices; always embracing a trauma-informed and person-centered approach.

Key Responsibilities:

  • Provide outreach and engagement to children and families.
  • Conduct a comprehensive, strengths-based assessment of the child and family and their service needs.
  • Develop a child and family specific plan of care in collaboration with each family and their care team.
  • Assist families in securing identified services and advocating to other systems for the provision of these services in an individualized and collaborative manner.
  • Facilitate the creation of a support network for each child and family; convene regular meetings with the family and care team to monitor progress towards jointly developed goals.
  • Facilitate ongoing communication among the child, family and community providers.
  • Provide crisis intervention and stabilization to children and families as needed.
  • Develop knowledge of and positive relationships with the multiple systems serving families and youth.
  • Provide 24-hour telephonic crisis intervention services on rotating basis.

Key Competencies:

  • Must be knowledgeable of behavioral health diagnoses and symptomatology.
  • Must complete the CANS-NY Training and be certified to use the CANS-NY.
  • Must be able to effectively engage clients and coordinate care with different providers.
  • Highly organized and self-motivated.
  • Familiarity with and ability to use an Electronic Health Record system.

Key Performance Indicators (KPIs):

  • Complete CANS-NY assessments at enrollment and annually.
  • Meet program productivity standards.
  • Complete documentation in a timely manner, as per Health Homes and agency’s standards.

Requirements:

  • Bachelor’s Degree with two years of relevant experience or be licensed as a Registered Nurse with two years of relevant experience or a Master’s Degree with one year of relevant experience.
  • Strong computer knowledge: typing, communication tools, scheduling tools and web browsers.
  • Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
  • Have and maintain a valid Driver’s License with a driving record that is satisfactory to our insurance carrier.
  • The Care Manager must maintain a current NYS Defensive Driving certification.
  • The Care Manager must be cleared NYS Justice Center through fingerprinting for a Criminal History Records search.
  • The Care Manager must be cleared by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State Medicaid Exclusion List.
  • The Care Manager must be cleared by the NYS Office of Children and Family Services (OCFS) for instances of child abuse and/or neglect.
  • The Care Manager must be legally eligible to work in the United States without sponsorship.
  • Fluency in English (Reading, Writing, Speaking).
  • Fluency in Spanish preferred but not required.

Special Considerations :

  • Hours per week, reporting structure, and location are subject to change per program needs.

Mandated Reporter:

This is a Mandated Reporter position. The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian” under the law and, therefore, a mandated reporter. You can be held liable by both the civil and criminal legal systems for intentionally failing to make a report.
